To Make a Referral

There are several options for how professionals can make referrals to us.

1. You can give our information to your patient or client.

a. Give your patient the Hoover & Associates Referral Form.

• If you are referring your patient to a specific clinician, please write that clinician’s name on the referral form, and instruct the patient to say so when they call us.
• Instruct the patient to contact us at 708-429-6999 to schedule an appointment.

b. Give your patient the Hoover & Associates “Meet Our Clinicians” handout.

c. If you prefer to give one of our other handouts or business cards to your patient, you can request paper copies to be mailed to you by calling 708-429-6999.

2. You can call us yourself at 708-429-6999.

You can contact our office at 708-429-6999 to make a referral. This may be especially helpful if:

• if you are concerned about the patient’s follow-through, and want to facilitate the scheduling of their first appointment while they are in the office with you.
• if you’d like to consult with us or ask us any questions before making the referral.
• if you’d like to make a referral to a specific clinician, and if you’d like to brief that clinician on the case.

3. You can fax us your patient’s information.

If you’d like us to contact your patient and schedule their first appointment, please fax us the Hoover & Associates Referral Form.
